Fees and Insurance

We have fees for different types of visits and tests that are performed.  We use a sliding fee scale.  Discounts are based on family size and income.  Our registration staff will be glad to explain our system to you when you come in for your visit.  We will not deny health services to anyone due to an inability to pay.

To apply for our Reduced Fee Program, you may print and complete the reduced fee application form by clicking one of the following lin. Please read the directions carefully and bring the completed form to your next appointment.

A Word about Deductibles and Co-pays

While all Family First Health centers accept most forms of insurance, some insurance plans require the patient to pay a deductible before they will pay for services with us.  These deductibles vary among insurance companies.  Even within an insurance company, different insurance plans may have different deductibles.  If your insurance plan has a deductible, you must pay that amount before the insurance company will make payments to us.  You are responsible for paying this deductible.

In addition, some insurance companies may require you to pay a co-pay for an office visit.  A co-pay is a small amount that you must pay at each visit to your medical provider.  The amount of the co-pay varies depending upon your insurance company and your plan.  If your insurance plan has a co-pay, we will charge you that amount at the time of service, and you are responsible for this fee.

To find out if your insurance plan has a deductible or a co-pay, you should contact the insurance company’s customer service department (the phone number can usually be found on your insurance card.)  If your insurance plan is offered through your employer, you can also find out about any deductibles or co-pays from your employer.

Healthy York Network

Family First Health is a participant in the Healthy York Network.  Healthy York Network is a group of health care providers and social service agencies working to improve access to health care by offering care and medications at a discounted rate and by connecting patients with medical coverage for which they may be eligible.  HYN is not health insurance.  Membership is based on financial eligibility.  There may be some geographic restrictions, also.

Other member organizations of HYN include WellSpan Health, Memorial Hospital, the York City Bureau of Health, York Spanish American Center, and the Community Progress Council.  HYN is funded by a grant from Health Resource Services Administration.

To learn more about Healthy York Network, please contact us 845-8617.

The mission of Family First Health is to provide compassionate and comprehensive health services to people seeking accessible and affordable quality care.

Payment Plans
