That is why Family First Health offers a variety of social services and community outreach initiatives to provide the caring, responsive assistance these individuals need.
Special Social Services Programs
- Caring Together — High-quality care for patients who are HIV positive.
- Connections for a Healthy Pregnancy — A free community outreach program serving York City and surrounding neighborhoods that helps to guide expectant mothers in need of resources for a healthy pregnancy.
- Healthy York Network (HYN) — A group of health care providers and social service agencies working to improve access to health care by offering care and medications at a discounted rate and by connecting patients with medical coverage for which they may be eligible.
- Nurse-Family Partnership — Prenatal and early childhood nurse visitation that improves the health and well-being of MA/WIC eligible first-time mothers and their babies.
