 Family First Health offers a compassionate team of social service providers who are committed to addressing the special needs of our patients. Issues in daily life can prevent clients from achieving optimal health and wellness. Family First Health offers a variety of social support services and community outreach initiatives to provide caring, responsive assistance.
Social Services: Certified Spanish translators on staff Counselors help with problems of daily living (English and Spanish) 24-hour support for the emergency health needs of our patients Nurse home visits to first-time eligible mothers Community outreach efforts Case management for people living with HIV and AIDS Healthy York Network screening and case management
Special Programs: Caring Together is a unique collaboration between Family First Health, WellSpan Health and OPEN (Ordinary People Extraordinary Needs) offering a comprehensive system of HIV/AIDS care. Together, we are a supportive network helping patients manage their medical care while addressing daily living needs. CLICK HERE for additional information.
Nurse-Family Partnership is a program for first-time eligible mothers and their children. The only program of its kind in York, Nurse-Family Partnerships specially trained nurses provide support, education and counseling on health and parenting issues. The program begins in early pregnancy and continues through the childs second year. CLICK HERE for additional information. |
Healthy York Network
Family First Health is a participant in the Healthy York Network. Healthy York Network is a group of health care providers and social service agencies working to improve access to health care by offering care and medications at a discounted rate and by connecting patients with medical coverage for which they may be eligible. HYN is not health insurance. Membership is based on financial eligibility. There may be some geographic restrictions, also.
Other member organizations of HYN include WellSpan Health, Memorial Hospital, the York City Bureau of Health, York Spanish American Center, and the Community Progress Council. HYN is funded by a grant from Health Resource Services Administration.
