Family First Health is a recognized Patient Centered Medical Home (PCMH) and provides team-based care with a focus on wellness and prevention. Our providers meet patients’ needs by providing ongoing, consistent preventative care and delivering the full range of services that patients need.
How can the Family First Health medical team help me?
The medical team including your provider, nurse, medical assistant and medical receptionist pre-plan for your visit and review what is needed to make sure your medical visit includes all aspects of care for good health outcomes. Your medical team will engage other care team members as needed, depending on your specific needs. This may be a referral to specialty providers, lab orders, schedule additional testing and in house referrals for guided care coordination or integrated services.
Family First Health is proud to offer board certified physicians and mid-level providers at all our community health centers. Additional specialty providers have been added to our George Street and Hanover Centers to offer patients expertise in the areas of pediatrics and infectious disease. All providers participate in the Chronic Care Model to manage a patient’s chronic diseases, such as hypertension and diabetes.

How do I make an appointment?
If this is your first appointment, please call the center of your choice and talk with our team of medical receptionists to schedule your first appointment. You will be able to see which of our providers are available to see new patients, and pick an appointment that works best for both of you. We have a full-range of medical doctors, doctors of osteopathic medicine, certified nurse practitioners, and physicians assistants. Please see our full list of providers here.

What should I know/bring for my first appointment?
- Photo ID/driver’s license
- Insurance card
- A list of any questions you may have for your provider and care team
- Name/address of recent health care provider (So we can send a signed release for medical records for your new provider to review with you)

Will all of my questions and health care needs be taken care of at my first medical visit?
As always, we will do our best to take care of your questions and health care needs at your first visit. If we need additional time to address these, we will schedule you for a follow-up visit as soon as possible.
How will I know if you accept my insurance?
When you call to schedule your first appointment, we will ask for your insurance information and should be able to confirm right away. If not, we have several options available, including our sliding fee discount. You may also check our full list of accepted insurances.
If I don’t have insurance, can I be seen?
Of course! We will see anyone, regardless of ability to pay. There are several options available including our sliding fee discount. If you are interested in looking for insurance through the Health Insurance Marketplace, we will schedule time for you to meet with our insurance navigator who can walk you through that process.
In addition to our full-range of services, our Guided Care Coordination program is a specific team dedicated to helping our primary medical and dental patients navigate and access community resources to improve their overall health. They are able to help with barriers that may affect your health, like access to food, housing assistance, transportation to medical appointments, and much more.
