Take the Test, Take Control: Family First Health recognizes National HIV Testing Day
with free, confidential testing open to everyone

WHAT
Family First Health will offer free, confidential HIV testing at its George Street Center on June 26 in recognition of National HIV Testing Day.

Individuals are encouraged to “Take the Test. Take Control,” and will be able to choose from one of many incentives, including coupons and food items from Healthy World Café, Rita’s Italian Ice, Subway, Gold’s Gym, Jimmy Johns, Wendy’s, Marcello’s Pizza, York City Pretzel Company and more. Participants can also enter into a gift basket raffle.

Staff and volunteers will also be distributing condoms and educational materials in downtown York to encourage individuals to get tested at the George Street center. Testing is open to everyone, not just Family First Health patients.

BACKGROUND
Centers for Disease Control recommends that health care providers test everyone between the ages of 13 and 64 at least once as part of routine health care. One in seven people in the United States who have HIV do not know they are infected. Family First Health provides free, confidential HIV testing at all five locations year-round, as part of routine medical care. In 2014, staff performed nearly 3,500 tests.

Family First Health celebrates 45 Years of Putting Community First
 

York, Pa. (8 June 2015) – Family First Health is thrilled to celebrate a major milestone in 2015 as it marks 45 years of providing health care in York County.

One of the first community health centers in the nation, Family First Health was founded in 1970 in response to the York Charrette, which identified solutions to several programs facing York City. Family First Health proved to be an effective way to meet the comprehensive health care needs of underserved populations.

In 45 years, Family First Health has grown to five locations in York and Adams counties. Its uniquely qualified providers delivered medical, dental and social services to more than 22,000 patients in 2014, furthering its goal of addressing barriers to better health and increasing affordable, accessible health care for those who need it most.

“Family First Health began several decades ago as a response to community need,” said Jenny Englerth, CEO. “We’re proud to carry that mission focus through every day in our work with individuals and families to effect community-wide changes and address the challenges they face in achieving better health.”

Family First Health will be celebrating this milestone anniversary all year, kicking off June 1 by sharing diverse stories of how the community health center puts community first in its program and services every day.

Family First Health earns national recognition for patient-centered care

 

Southcentral Pa. (3 June 2015) – Family First Health is pleased to announce it has earned National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition for its use of evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

Family First Health received accreditation for all five of its community health centers, in York, Hannah Penn, Lewisberry, Hanover and Gettysburg.

“Since its start 45 years ago, Family First Health has been dedicated to relationship-based care that addresses all facets of health and improves the health of our community as a whole,” said Family First Health CEO Jenny Englerth. “We are honored to be recognized by NCQA, which raises the bar in defining high-quality care.”

To earn recognition, which is valid for three years, Family First Health demonstrated the ability to meet the program’s key elements and embody characteristics of a medical home. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

Family First Health, a community health center serving York and Adams counties, invites the community to “Get Movin’ with Dr. Go” and improve your health one step at a time. Dr. Asceline Go, the medical director for Family First Health, will lead a 30-minute group walk on the first and third Wednesdays of each month, departing from the George Street Center, 116 S. George St., York, and heading toward York City’s Penn Park. Groups will meet in the lobby of the Family First Health George Street Center, and the walks are open to all fitness levels. Family First Health receives $20,000 grant from the Hahn Home Fund of the York County Community Foundation for care at the White Rose Senior Center

York, Pa. (6 May 2015) – Family First Health is pleased to announce it has received a $20,000 grant from the Hahn Home Fund of the York County Community Foundation for support on delivering primary care to members of the White Rose Senior Center. The goal is to increase access to primary care while improving the health of older adults in the City of York.

As a federally qualified community health center, Family First Health recognizes that healthy living goes beyond the walls of its health centers. Senior centers are often a focal point for aging services in the community, with more than 100 older adults spending much of their day participating in activities at White Rose Senior Center.

Through this grant, Family First Health will implement the “hub and spoke” primary care delivery model to utilize nurse practitioners and physician assistants to deliver on-site health needs assessments and primary care services to increase access to care and connect those who need it to a medical home. Services are slated to begin in July 2015.

On-site providers will be able to treat individuals for everything from the common cold to managing chronic conditions, as well as administer educational programming that encourages healthy habits. Such use of the grant embodies the Health & Well-Being and Community Culture Change priority areas in the Embracing Aging Grants Program.

FOX43: Family First Health kicks off Eat Your Greens

Kids throughout Central Pennsylvania got in the St. Patty’s Day spirit while they also learned a lesson on eating healthy. Family First Health kicked off its “Eat Your Greens” campaign. The family friendly event took place Saturday at Central Market. Kids created festive leprechaun masks while adults picked up free recipe cards for healthy snacks, like veggies. The event’s a way to encourage patients to make fruits and veggies part of their discussion with doctors and dentists.

Family First Health launches ‘Eat Your Greens’ with ideas to sneak veggies on the go

Central PA (4 March 2015) — Family First Health, a community health center in York and Adams counties, is encouraging patients to make fruits and veggies part of the discussion with their doctors and dentists. Through a series of events called “Eat Your Greens,” Family First Health is encouraging a healthier lifestyle that improves the overall health of our communities.  

The campaign kicks off with a family-friendly event from 10 a.m. to 2 p.m. Saturday, March 14, at York’s Central Market, where kids can create a festive leprechaun mask and adults can pick up free recipe cards for veggie-friendly snacks. Family First Health will also be partnering with Hively Landscaping to give away reusable shopping bags to hold all your goodies!

Family First Health will also sponsor the first-ever vegetarian chef challenge at YorKitchen on March 26 with Chefs Tim Spangler of Mezzogiorno and Trae Grabosch of LettUsKnow. The campaign wraps up with Family First’s famous stuffed veggies at a photo booth in Central Market for “Go Green in the City” April 18. Stop in and pose with your pledge to #EatYourGreensYork.

In the campaign’s third year, Family First Health is focused on the message that incorporating vegetables into your meals and snacks doesn’t have to be time-consuming or difficult. The public can visit familyfirsthealth.org/greens for easy ideas on how to eat healthfully while on the go, or Like Family First Health on Facebook and follow them on Twitter for shareable images and quick tips throughout March and April.
